Position: Licensed Social Services Director

Job Summary:

Work exclusively with Alzheimer’s/Dementia population and their respective families. Responsible for identifying and providing each Resident’s social, emotional and psychological needs for the continuing development of the Residents’ full potential; assists with planning for Transfer and Discharge; must exhibit a sincere desire to work with the elderly and communicate courteously with Employees, Residents and others

Qualifications and Skills:

Experience and/or specific training related to Alzheimer’s/Dementia is required. Prefer Bachelor’s Degree or Master’s Degree; preferably Licensed in the State of MS in Social Work, or two years Social Services experience in Long-Term Care. Candidate will possess an understanding and in-depth knowledge of Alzheimer’s/dementia, aging and cognitive decline, and communication with dementia residents and their families. Candidate must have basic computer skills in Microsoft Office, excellent oral and written communications skills and have a valid Mississippi Driver’s License; must pass drug screens and have a satisfactory Criminal History Background Check

Duties and Responsibilities:

  • Develops Social History, Social Assessment, Care Plan to identify problems, needs, set realistic goals, problem resolution and/or needs upon Admission, compliant with State and Federal Regulations
  • Participates in interdisciplinary Care Plan Meetings and in Continuous Quality Improvement (CQI) Program
  • Documents Progress Notes for Resident’s Care Plan when necessary within Company Policy time frames
  • Assists Residents and family members with resident transfers, discharge planning and discharge process
  • Promotes communication assistance and support for family members and Residents; provides assistance with TDD phones and/or other communication assistance for impaired Residents
  • Attends public relations activities as requested by the Administrator and assists with community education regarding aging, Facility services and other related information
  • Participates in pre-admission screening of potential Residents and with providing information to Residents and families during the admissions process
  • Communicates with state and local regulatory agencies in regards to benefits such as Medicaid, Ombudsmen, PASARR
  • Orients Residents to long-term care environment, Facility policies and Facility’s staff
  • Interviews, orients, trains, supervises and evaluates Social Services personnel; makes work assignments, sets goals and priorities; keeps associates informed of all changes
  • Manages ordering of supplies; maintains records for budget and supplies
  • As requested by Administrator, attends Family Council and Resident Council Meetings; counsels with families
  • Learns and complies with all Company policies, State and Federal Regulations regarding Social Services
  • Must possess a valid Mississippi driver’s license; assists with transporting Residents to doctor’s appointments and shopping as needed
  • Ensures Residents’ personal property lists are updated at least annually
  • Cooperates with and assists the Activity Department with Residents’ activities
  • Ensures compliance with and completion of the MDS, CAT’s and CAA’s within pre-set time frame and criteria
  • Attends trainings and relevant educational opportunities as directed by Administrator.
