Nehalem Bay Health Center and Pharmacy, located on the beautiful North Oregon Coast, has an immediate opening for a full-time or part-time Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW). Job Purpose:

The LPC or LCSW is responsible for providing confidential addictions counseling, education, and support to patients and their families and for promoting healthy lifestyles and healthy choices for these individuals and the community.

Duties:

  • Conducts full biopsychosocial assessments and develops treatment plan.
  • Uses the DSM-V to diagnose substance use disorders.
  • Works in collaboration with the patient and members of the treatment team to develop a comprehensive treatment plan that addresses priority needs of the patient and assists patient with goal setting that supports sobriety.
  • Collaborates with Medical Director and other Providers when necessary.
  • Monitors patient progress and suggests changes to the therapeutic approach and best practice, evidenced based interventions based on patient response to treatment.
  • Monitors, records, and observes clients taking medication, as needed.
  • Conducts drug screenings.
  • Oversees caseload.
  • Conducts/facilitates process, psycho-educational, relapse prevention, and other groups as assigned.
  • Provides counseling to individuals, including pregnant women, regarding the use and effects of alcohol and drugs on client/fetus.
  • Provides community education and support programs, as requested by Clinic leadership.
  • Documents in the patient’s medical record within 24 hours of the patient being seen.
  • Documents the assessment of client progress/regress and interventions provided in the medical record, on an ongoing basis.
  • Communicate and coordinate with any necessary outside sources, such as mental health treatment providers.
  • Conducts individual and family sessions.
  • Recognize, respond to and assist in psychiatric crisis situations and provide the intervention necessary.
  • Attend clinical and staff meetings as required.
  • Works with community agencies as needed.
  • Accesses and collects relevant information and resources, assures clinic has all releases of information signed, dated, and current.
  • Maintains strictest confidentiality; adheres to HIPAA guidelines/regulations.
  • Maintains patient records related to screening, treatment plan, continuing care, progress toward goals and objectives, discharge summary, and treatment outcomes that are accurate, concise, timely, and useful for coordinating treatment team planning, monitoring, and review.
  • Coordinates and cooperates with local agencies and organizations necessary to expedite treatment for each patient.
  • Participates fully as a member of an interdisciplinary treatment team.
  • Provides treatment referrals via Clinic Referral Coordinator.
  • Assists individuals to identify their need for treatment programs.
  • Performs in a professional manner and role models a positive image of substance abuse.
  • Follows policies and procedures.
  • Performs other related duties as assigned.

Skills/Attributes:

  • Understands neonatal abstinence syndrome.
  • Demonstrates knowledge of chemical dependency and addictions and utilizes knowledge and skills to access and meet the client’s treatment needs and requirements.
  • Ability to work as a team member, demonstrating maturity under stress and objectivity.
  • Demonstrates good communication skills.
  • Can utilize knowledge of a variety of treatment models and theories to understand and plan intervention strategies for a variety of patients.
  • Can write sound clinical notes.
  • Can perform COWS.
  • Demonstrate good decision making in dealing with safety issues related to aggressive and/or intoxicated clients, as needed.
  • Strong clinical mediation and negotiation skills.
  • Ability to prioritize multiple tasks and perform efficiently and effectively in a stressful environment.
  • Ability to work well in fast-paced setting.
  • Ability to make accurate clinical assessments of patient, through observation and interview.
  • Empathy towards and understanding of patients with special needs.
  • Ability to work effectively with people from diverse social, cultural and ethnic backgrounds.
  • Ability to proficiently and accurately read and write in English.

Qualifications:

  • Active Oregon LPC or LCSW License.
  • Active Oregon CADC II certification, or to get CADC II within 1 year.
  • Minimum 2 years in primary care, hospital, or addictions setting.
  • Maintain license and continuing education in accordance with State, Federal, and Regulatory agencies’ rules and regulations.
  • Knowledge and experience working with Medication-Assisted Treatment (MAT).
  • Ability to relate well to patients and staff.
  • Current CPR certification.
  • Ability and willingness to adhere to workplace expectations as outlined in employee handbook.

Physical Requirements:

  • Ability to communicate both in person and by phone.
  • Ability to lift up to twenty-five (25) pounds.
  • Ability to sit for long periods of time.
  • Manual dexterity when inputting computer information.
  • Exposure to normal noise, temperature, and dust and fume levels.
