Licensed Practical Nurse (LPN) Part-time & Contract Position

Job Description: We are currently hiring a reliable and compassionate Licensed Practical Nurse (LPN) who wants to make a difference in our clients’ lives. We currently have a job opening for Licensed Practical Nurse (LPN) in the Charleston, Dorchester, Berkeley and Colleton Counties.

As a home care nurse, you will be an integral member of a multi-disciplinary health care team that provides non-skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities of LPN:

*Making home visits to clients living in designated territories *Performing assigned duties *Monitoring clients’ conditions; reporting changes to the Case Manager *Following a written Plan of Care and documenting in the EMR per agency policy *Adheres to all Agency documentation policies and procedures *Training and education of family members *Communication with other members of the patients’ multi-disciplinary team *Supply management *Emergency management *Attends all staff meeting and training

Qualifications of Licensed Practical Nurse (LPN):

*Experience in home care and/or home health preferred *Current license as a Licensed Practice Nurse in the state of South Carolina *Minimum of 2 years experience *Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma *Current CPR certification *Fluency in English is required *Negative TB test or chest x-ray *Must be competent using an EMR *Must be able to complete documentation within 24 hours *Ability to work independently and manage time effectively *Strong interpersonal skills *Solid computer skills; prior experience with EMR preferred *Ability to travel to cases as assigned with valid, unrestricted driver's license and your own vehicle with proof of auto insurance and registration from DMV *Ability to pass background check

LPN Home Care Pay:

*Based on prior experience *Flexible hours *Mileage paid

Available shifts and compensation: We have available shifts all days of the week. Compensation is $24.00 - $27.00/hour.
