Job description
Under the direction of the RN Supervisor, provides nursing care to meet the nursing needs of the residents in accordance with Woodcrest Rehabilitation's established policies, nursing procedures
  • Assumes responsibility of Charge Nurse
  • Receives report at the beginning of the shift and gives report to on-coming shift
  • Count and record narcotics at beginning and end of shift and secures the keys.
  • Ensure that the narcotic counts and records are current at all times
  • Organizes nursing care for all residents on unit.
  • Makes rounds of unit at beginning of shift and periodically to assess resident’s needs and monitor environment for optimum comfort and safety of residents, staff and visitors.
  • Completes and reviews daily Certified Nursing Aids assignment
  • Gives report to CNA’s and includes pertinent information and changes in resident status and condition
  • Ensures the CNA’s complete all documentation by the end of the shift
  • Observes residents for changes in physical, mental and or emotional conditions and reports to the nursing supervisor
  • Notifies RNS immediately of emergency situations and starts emergency measures as needed
  • Assists the RN Supervisor in care planning such as adding episodic events to the care plan.
  • Participates in gathering data on residents upon admission to provide information from which a nursing care plan can be developed
  • Ensure that restorative and rehabilitative plans are carried out
  • Admits, transfers and discharges residents as necessary
  • Ensure that established infection control policies and procedures are followed at all times
  • Ensure during discharge home that the resident is escorted to the Lobby area
  • Documents pertinent information on the resident’s chart and care plan
  • Completes required record keeping forms/charts upon the resident’s admission, discharge, and transfer. Ensure forms are filed in resident’s medical record and or forwarded to the appropriate area
  • Takes and records vital signs and reports abnormalities
  • Administers medications and treatments as per physician orders and in compliance with medication and treatment policies and procedures: secures medication cart
  • Ensures that medication and treatments are given to the right resident by checking that all residents have the appropriate identification as in wristbands before dispensing medication or treatment
  • When administering medication, remain with the resident to ensure that he/she swallows or takes medications as instructed
  • Charts medication and treatments administered and resident’s reaction to same.
  • Picks up and transcribes physician orders
  • Reviews physician orders for completeness for the monthly visit including the interim orders
  • Fax drug orders to the pharmacy immediately
  • Assist physicians with examination and treatment of residents
  • Ensure that residents are scheduled for diagnostic and therapeutic services as ordered by the physician in accordance to policy and procedure
  • Implement and maintain nursing objectives and standards.
  • Assure that nursing personnel, residents, private duty companions/ CNAs and visitors follow established policies and procedures
  • Ensure that each resident’s right to fair and equitable treatment, self-determination, privacy, property and civil rights, including the right to complain, is followed by all department personnel
  • Create and maintain an atmosphere of warmth, personal and positive emphasis, as well as a calm environment throughout the unit
  • Maintains confidentiality of all pertinent resident information at all times
  • Coordinates nursing services with other departments
  • Assures that unit supplies are maintained at adequate levels.
  • Work with consultants as necessary and implement changes as needed
  • Assists CNA’s in the event of the death of a resident to ensure that the body is properly prepared for pick-up by the funeral director/ morgue
  • Ensure that the assigned work area (nurse’s station, medication room, etc.) are maintained in a clean, safe and sanitary manner
  • Complete Accident/Incident reports on all occurrences and chart information in the medical record as per policy and procedure
  • Notify the nursing Supervisor and the Director of Nursing when a resident is discharged Against Medical Advise (AMA) and ensure that proper documentation is done
  • Report all discrepancies to the Nursing Supervisor
  • Report all hazardous conditions and equipment to the maintenance department immediately
  • Participates in facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing
  • Must participate as needed during investigations into allegations of abuse neglect or mistreatment
  • Must cooperate with government agencies in the event of an investigation into resident abuse
  • Attends mandatory in-services annually and any other in-services as deemed necessary
JOB COMPETENCY:
  • Must be a graduate of an accredited LPN nursing program
  • Current registration and license to practice as a LPN in the State of New York
  • Must be CPR certified
  • Must have a minimum of one year in a skilled nursing facility or other medical facility
  • Must be able to read, write and speak the English language
  • Must posses the ability to make independent decisions when the circumstances warrant such actions
  • Must posses the ability to deal effectively with emotional residents, visitors or family members
  • Must be a minimum of 20 years of age
  • Must be knowledgeable of nursing and medical practices and procedures as well as laws, regulations and guidelines pertaining to long term care
