Job Title: Nurse Practitioner
Payrate: $56.14
Location: Hilliard FL 32046 & Yulee, FL 32097
Schedule: Mon-Fri 8AM - 5PM EST
Term: 3 Months High Potential to Extend
Vaccine Requirement – Vaccine is required

Responsibilities:

• The Nurse Practitioner will evaluates and treats center patients in accordance with standards of care.
• The Nurse Practitioner will follows level of medical care and quality for patients and monitors care using available data and chart reviews.
• The Nurse Practitioner will assists in the coordination of patient services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care.
• Completes all medical record documentation in a timely manner working with a quality- based coder to optimize coding specificity.
• Helps Regional Medical Director (RMD), Physician and Center Administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor.

Required Skills & Education:

• Successful completion of a Nurse Practitioner program with Board Certification
• Active, unrestricted Nurse Practitioner license in state of practice location
• Basic computer skills, including email and EMR.
• Appropriate certification to write prescriptions under the authority of a collaborating physician/medical director
• two to five years directly applicable experience preferred
