**This can be a remote position, but the successful candidate should live within driving distance to any one of our sites in Miles City, Hardin, or Glendive . Occasional travel may be required.**

One Health is looking for a Montana-Licensed Professional Counselor OR Montana-Licensed Clinical Social Worker to join our Behavioral Health team working with students, families and community members in the Miles City area. There is high, critical need for behavioral health services among our youth, and One Health is looking for the right person with a passion for helping others! This is a full-time position in a school-based and clinical setting. This is not a school counselor position.
POSITION SUMMARY:
Responsible for providing behavioral health services to One Health patients in a school-based setting, either within a school-based health clinic or office that One Health oversees. Space is generally provided by the school. Provides, oversees, and/or administers a wide range of psychosocial evaluation/assessment, diagnostic, counseling therapy, and/or case management services, requiring a high degree of independent decision-making and program administration. Leads and trains other staff engaged in related case management activities, as appropriate to the individual position. Will utilize a balanced approach which includes perspectives of all disciplines involved in health services and will have a role definition flexible enough to accommodate the diversity of programs offered.

ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:

- Provides counseling, therapy, and/or psychotherapy to patients/students (including walk-ins) within licensed scope of practice; evaluates and treats mental illness and substance abuse, treats or appropriately refers patients for specialty evaluation and treatment.
- Prepares treatment plans, discharge plans, and follow-up care programs; provides preventive health/lifestyle education, and case management.
- Engages in collaboration and consultation with staff.
- Collects data about patients/students through interview, case history, screenings, and/or observational techniques; evaluates data to identify risk factors to determine proper therapeutic approach or referral to specialists.
- Makes appropriate referrals for financial assistance and other needed services.
- Provides ongoing assessment, using screen tools as appropriate, to modify treatment plan as indicated.
- Follows One Health Policy related to documentation in EHR, release of information, subpoenas, and record keeping. Consults with legal, director of Behavioral Health, School Based Health Care, and/or Risk and Compliance/Privacy as needed.
- May plan or facilitate outreach and education activities, as needed and appropriate to the position;
- Participates in multi-disciplinary, team-based care of patients/students including work with school staff.
- Provides consultation to other members of primary care and behavioral health care teams.
- Participates in training, reporting, and other requirements of various grant supported programs implemented by One Health and partners.
- Participates in the student and/or other clinical discipline training activities.
- Participates in the formation and evaluation of applicable protocols, workflows, policies/procedures for school- based services.
- Maintains continuity of care when health center patients utilize other health care facilities, i.e. referrals, emergency room services, outpatient departments, specialty clinics, and inpatient services.
- Participates in staff meetings, including quality assurance and behavioral health audit activities in keeping with health center procedures.
- Participates in school meetings, based on program or funding requirements as required.
- Participates in on-site and off-site outreach and continuing education programs.
- Participates in performance optimization and risk management activities as required.
- Adheres to all One Health Policies and Procedures.
- Cooperates and abides by all elements of One Health’s Compliance Program.
- Provides exceptional customer service and models the mission of One Health when interacting with internal and external stakeholders promoting a positive image for the health center.
- Exemplifies a patient-centered care approach to each patient along each stage of the patient journey, while working as a key patient centered team member by communicating individual patient care needs to support the needs of patients served.
- Other duties may be assigned as required or needed with or without prior notification.
MINIMUM JOB QUALIFICATIONS:
Education/Certificates/Licenses:
Required
- Master's Social Work (MSW) and licensed in the State of Montana as a LCSW, OR
- Master's in Counseling and licensed in the State of Montana as a LCPC
- BLS Certification upon hire
- Valid driver's license
Preferred
- Dual Licensure in the State of Montana
- Substance Use Disorder certification/licensure
Experience:
