Aventura at Walton Hills is looking for an Interim Social Service Director for our wonderful SNF in Walton Hills, OH. The ideal candidate is one that is looking for a challenge to take on, expect to tackle it, and be rewarded for overcoming it. The ideal candidate will have prior experience in the social work field. The social worker will assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Qualifications/ Responsibilities:
- Must have, at a minimum, one (1) year supervised social work experience in a health care setting working directly with individuals
- Long Term Experience Required
- Participates in discharge planning, development and implementation of social care plans and resident assessments
- Interviews residents/families to obtain social history
- Reviews departmental complaints and grievances from residents and personnel and prepare written reports to Administrator of action(s) taken
- Ensures that all charted progress notes are informative and descriptive of the services provided and of the residents response to the service
- Coordinate resident/family in planning of objectives and goals for the resident
- Administrative duties to include but not limited to submission of necessary forms and reports
