Our organization is seeking an Intake Coordinator to join our team. The successful candidate will be responsible for managing the intake process for new clients, including collecting and verifying information, scheduling and conducting tours, and providing general support to ensure a smooth admission experience. The ideal candidate will have excellent organizational skills, a strong attention to detail, and the ability to effectively communicate with clients and other stakeholders.

TASKS AND RESPONSIBILITIES:

• Facilitates assessment of general intake from potential clients or their referral sources
• Prepare pre admission package for all potential clients
• Verify insurance
• Communicate with doctors , insurances and ASAP
• Obtain all necessary paper work for a potential client to be send to SCO, ASAP, etc
• Responsible for initial admission
• Evaluate social and community needs of participants and families
• Interact with patients
• Maintain master list of clients
• Generates required reports
• Collaborate with team members and referring professions to provide positive seamless services
• Maintain list of Community Services available for seniors
• Assist participants with securing social and medical services through community agencies as needed as well as referring clients to available community services
• Document and follow up all community services received by the participants
• Assist as necessary in delivery of other required program services
• Provide assistance as detailed in interdisciplinary care plan
• Understand scope of practice of RC role and perform within parameters of job description
• Understand RC role in Fire and Disaster Procedures
• Pro-actively identifies the needs of each participant and effectively provide services
• Documents participant’s needs and services provided in report form
• Continue to identify the social needs of participant throughout their participation in the day program and in the home
• Effectively serves as the patient advocate in collaboration with patient’s health care providers and community resources
• Assist participants in obtaining necessary medical services in the community not available through the day program (such as vision care, podiatry, medical equipment, etc.)
• Completes accurate and timely documentation of all services

QUALIFICATIONS:

1. Academic: High School diploma or equivalent

2. Experience: Working in adult care setting

3. Health: Pre-employment clearance by a physician that certifies the employee to be free of infectious disease and/or restrictions that would inhibit performing job related responsibilities

4. Language: Able to read, write and speak English.

Job Type: Full-time

Pay: $17.00 - $19.00 per hour
