The In Home Outreach Counselor (or IHOC) provides in-home counseling, counseling, direct supportive, crisis intervention, and linkage services to DCFS referred families. The position requires ongoing monitoring and assessing of the therapeutic dynamics of families, assisting in case treatment planning, coordinating the overall case management needs and ensure the service-related program goals and activities are meeting the special needs of children and families.

The IHOC also serve as a liaison for clients and their families assisting with medical, mental health, school and other public social services. The IHOC maintains client records and required reporting documentation and necessary forms, monitors follow up, and evaluates family participation in the program. There is required collaboration and the development of relationships with Children’s Social Workers, DCFS, DMH, Department of Probation, and other Network and community agencies.

Master's Degree preferred from an accredited school in the field of social work, counseling, psychology, or closely related field. Bachelor's degree in Social Work, Psychology, Human Services or related field with two years of experience working with children and families in providing direct services including; case management, therapy, crisis intervention, and case planning in both community agency and in-home settings.

Knowledge of and experience working with multi-ethnic communities.
