Opportunity Knocks Opportunity Knocks is a volunteer mental health program providing outpatient services for individuals between the ages of 18-59, with severe and persistent mental illness, recent history of incarceration due to mental illness, and are currently homeless or at risk of homelessness. Our services include intensive case management, psychotherapy, skill-development groups, medication management, vocational linkage, temporary housing support, benefits processing, and outreach and engagement.
Pathways is seeking an innovative and purpose driven individual to join our team as a Full-Time Housing Specialist for our Catalyst program in San Diego.Catalyst is an Assertive Community Treatment (ACT) program. Catalyst provides full service partnership services to Transitional Age Youth (TAY) ages 16 to 25 with a serious mental illness, and who are homeless or at risk of becoming homeless in San Diego County. Catalyst’s ACT team provides comprehensive, individualized services in an integrated and continuous manner. Services are collaborative and recovery-oriented, and treatment includes: psychiatric assessment, medication management, individual therapy, substance abuse treatment, co-occurring disorders treatment, supportive housing, resource linkage, advocacy, family psycho-education and therapy, supportive employment and education development, legal assistance, community integration services, peer support and group counseling. SUMMARYThe Housing Specialist works with clients who are in need of housing solutions. They are responsible for outreach and networking within the community in order to maintain an up-to-date record of available housing resources.

ESSENTIAL DUTIES/RESPONSIBILITIES
- Work with Treatment teams to assess housing needs of clients
- Disseminate housing information for Treatment teams
- Research and inventory local housing options
- Work with Treatment team to determine housing needs and solutions for clients
- Maintain and ensures HIPAA, ethical standards and professional boundaries with all staff and clients
- Other duties as assigned or necessary to support the program and/or the company.
- Research and utilize our industry’s best practices and analyze our own services to ensure the best possible outcomes.
- Provide the best customer care possible.
- Identify and build upon the strengths of consumers, coworkers, and the communities we serve.
- Support consumers’ steps towards Recovery and Wellness.
- Create an organizational culture that respects and celebrates the diversity of our consumers.
- Value learning as an ongoing process that enables us to better service our consumers and establishes our leadership in the industry.
- Under moderate supervision of Clinical Supervisor
- Knowledge and experience with supported housing services for clients with mental illnesses.
- Excellent organizational, interpersonal and communications skills required.
- Must have intermediate computer skills
- Bachelor's degree from an accredited university in social services or related field with at least two years previous experience.
- Proof of: valid California driver’s license and auto insurance, as well as proof of education are required
- Office Environment (not remote position). Requires extensive sitting with periodic standing and walking;
- May be required to lift up to 20 pounds
- Requires significant use of computer, tablet, phone and/or general office equipment
- Needs adequate visual acuity, ability to grasp and handle objects
- Needs ability to communicate effectively through reading, writing, and speaking in person or on telephone
- Will require off-site travel 20% - local and out of state (meeting, county and/or client needs)
