Hanover Township Aging Services Department is accepting applications for a Home Care Coordinator. Under the direction of the Director of Aging Services, this position is responsible for the administration, coordination and service delivery of the Senior In-Home Care Program. This position will identify, evaluate, plan and coordinate individualized care and services for older adults in their homes. The Home Care Coordinator will work with an interdisciplinary team to coordinate care, identify and refer clients to appropriate resources and direct Home Care Assistants. This position will also communicate and build relationships with community organizations to facilitate referrals into the Home Care Program.

This position is non-exempt with a pay rate of $23.00 per hour, plus an excellent benefits package including major medical coverage, vision, dental, life insurance, and retirement through the Illinois Municipal Retirement Fund.
