Position Description
Under general supervision by the Senior Program Manager, the Senior Program Specialist will work on CKD Prevention initiatives with health systems, payers and healthcare providers to identify patients at greatest risk for developing chronic kidney disease; increase CKD testing for at-risk individuals; improve knowledge, awareness and understanding of preventable kidney disease and COVID-19 risk factors among providers and the general public; and identify financially sustainable strategies for kidney disease detection and management. The Senior Program Specialist will work with other programs as directed. Maintain and develop partnerships across the health care space with a high level of professional expertise.
Major Job Duties and Responsibilities:
- Implement CKD screening initiatives.
- Educate Michigan health plans and primary care providers about strategies to promote early detection of CKD. Provide technical assistance to support health plans and health systems/clinics in the implementation of strategies to facilitate early detection and management of CKD.
- Lead efforts to establish and maintain relationships with primary care providers and other health care providers to refer to NKFM programs, including through health system electronic health records.
- Identify best practices for CKD interventions and use of data to drive improvements in identification and management of CKD.
- Develop and maintain a positive working relationship with internal colleagues, volunteers, funders, and partners through clear communication and delivering as promised.
- Collect data, write reports and provide feedback to ensure meeting grant/contract deliverables.
