Position Overview

To support the mission and vision of Five Rivers Health Centers, the Health Equity Coordinator will address the root causes of health inequities through the assessment, planning, implementation, and evaluation process. The Health Equity Coordinator will further build internal organizational capacity; collaborate internally to connect health practice to social justice; engage community partners; develop partnerships and coalitions; design and implement strategies; and develop effective communication efforts.

Using a racial equity framework, a person in this position will work with the leadership team to develop organizational goals and objectives to address, reduce and eliminate racial health disparities and inequities. The Health Equity Coordinator will assist in the design and coordination of organizational changes to enhance activities related to the elimination of health disparities and inequities. A person in this position will develop and monitor an action plan to normalize organizational change to advance racial equity. A person in this position will serve as a resource for the organization on matters of racial and health equity, participate various department and community work groups on racial and health equity and monitor the progress toward achieving the department and community goals relative to racial and health equity.

General Responsibilities

• Responsible for the overall implementation and management of assigned prevention programs and activities to reduce health disparities in assigned patients.
• Develops and maintains collaborative partnerships internally and with other community organizations that address the same or related service population(s)
• Serves as a liaison internally and with organizations using program services to monitor and evaluate ongoing needs and effectiveness of programs and staff support.
• Analyzes data to identify workforce training and education needs.
• Lead, facilitate and/or advise performance improvement teams.
• Other duties as assigned

Qualifications

• High School diploma or equivalent required
• Windows-based computer experience required
• Associates Degree in the healthcare or business field preferred
• 2-3 years of health/social services experience preferred
• Certificate in Health Equity & Health Disparities preferred
• Experience with EPIC, Azara and Care Messenger preferred
