POSITION SUMMARY: The Eligibility Specialist is responsible for assisting patients with Siskiyou Community Health Center's Eligibility Determination process.

DUTIES & RESPONSIBILITIES:

· Identify patients without insurance and contact them by mail or phone to inform them about Siskiyou Community Health Center's eligibility determination process.

· Assist individuals with completing applications for the Oregon Health Plan and the Sliding Fee Discount Program.

· Answer questions from individuals, patients, and co-workers regarding the Oregon Health Plan and the Sliding Fee Discount Program.

· Communicate with Oregon Health Authority (OHA) to determine the status of an application.

· Verify insurance coverage with Oregon Health Plan and set up the insurance information in the patient's chart.

· Attend community events to promote the Oregon Health Plan and Siskiyou Community Health Center's eligibility determination process.

· Work with community partners to identify individuals who may benefit from enrollment services.

· Attend required meetings/trainings and share information with coworkers.

· Confidentiality, punctuality, excellent attendance, and ethical conduct are required.

· Other duties as assigned.

QUALIFICATIONS, EDUCATION, and EXPERIENCE:

· High school diploma or equivalent.

· Computer competency, including word processing and data entry.

· Effective customer relations skills, demonstrating the ability to work with diverse groups in a stressful environment, and displaying an understanding of group dynamics.

· Strong organizational skills and effective use of time.

· Ability to prioritize work and stay on task with little supervision.
