As a Case Manager, you will:
- Assist the treatment team in the assessment of each youth to develop an individual treatment plan.
- Coordinate and implement all aspects of the treatment plan to ensure compliance as defined by State of New Hampshire Family Centered Early Supports and Services.
- Provide and/or coordinate family case management to include meetings, transportation, respite, emergency services, and behavior management.
- Assist the family in finding resources for preventative and support services.
- Ensure participation in integrated community activities on a regular basis. hiring, supervisions, attendance, monthly financial reports and fire drills.)
- Draft all monthly reports and coordinate all treatment plan reviews and revisions.
- Establish and maintain relationships with providers (i.e. therapeutic services) for clients.
- Promote and maintain satisfactory public relations between Easterseals ESS, other programs, funding agencies and the community.
- Attend client-centered conferences, and team, agency and staff meetings.
- Bachelor's Degree in social work, psychology, education, nursing or a related field with emphasis in human services and family systems OR
- Associate’s Degree in the fields as above with four (4) years of human services experience
- Valid driver’s license and reliable transportation
Compensation: $45,000 a year with Bachelors degree; $48,000 a year with Masters degree
