Provide members non-emergent medical transportation with a company provided vehicle to their necessary health-related appointments as identified in the care plan established by their Care Manager. This role will be providing transportation services in Oneida, Madison, and surrounding counties.

Responsibilities:

  • Transport members to/from appointments in a courteous, safe, and timely manner. Driver will assist members in getting to and from their homes or appointment locations (this may include physical escort from the entranceways).
  • Accurately record mileage, communicate effectively with transportation coordinator regarding pickup/deliver, service, and client issues. Follow schedule, determining member and any assistive device requirement, checking road conditions, and estimating travel times.
  • The driver is responsible for the safe and comfortable access of the vehicle by all passengers, ensuring those wheelchair users and those unable to manage steps due so safely. All wheelchairs and equipment must be secured safely before driving.
  • Driver will pick up schedule daily; and report any changes to the schedule to the transportation coordinator immediately.
  • The driver will be responsible for daily safety checks of the vehicle. The driver will be responsible for reporting and scheduling with the transportation coordinators routine maintenance (e.g., oil changes, inspections, repairs) of the vehicle they are assigned. Driver will be responsible for the interior and exterior cleanliness of the vehicle they are assigned.
  • All vehicles must be returned upon completion of shift to VNA Homecare’s designated company parking lot or at another remote location as specified by management.
  • Driver must report incidents and/or accidents to the office immediately. An incident report must be completed within 24 hours of the incident.
  • Adhere to VNA Homecare Options Policies and Procedures regarding the care, safety, and delivery of services provided to our members.
  • Driver will attend any scheduled staff meetings as requested.
  • Driver will accept any other duties as assigned by the transportation department.

Requirements:

  • The driver must be over 21.
  • Driver must have a clean NYS driver’s license.
  • Excellent communication skills, compassion, and understanding of the aging process.

Physical Requirements:

  • Driver must be able to safely move objects of 40 pounds or more.
  • Ability to work under time-sensitive or stressful situations.
  • Remain seated in car for long periods of time.
  • Bloodborne Pathogens Exposure Category II- Normal work routine does not involve exposure, but exposure may be required as a condition of employment. Protective measures should be readily available.
