Overview: Provides administrative and clinical direction to achieve Mahaska Health’s population health strategic goals. Accountable for overseeing the management of performance metrics. The Director works closely with a variety of stakeholders, coordinating the activities of Population Health across the organization.
Responsibilities: ESSENTIAL JOB FUNCTIONS:

  • Provides leadership and direction in accordance with organizational strategic goals and objectives
  • Actively seeks opportunities to improve financial outcomes, engaging staff in the process
  • Develops, monitors, and communicates key metrics to support improved care and cost outcomes
  • Maintains knowledge of current and emerging population health strategies, models of care and related trends.
  • Has primary accountability for value-based care services
  • Acts as primary liaison between the Accountable Care Organization (ACO) and Mahaska Health
  • Leads and/or serves on committees to represent population health
  • Operationalizes and ensures that all population health related programs and initiatives meet regulatory and contractual requirements.
  • Works closely with other departments to train and support providers and clinical staff on the successful execution of value-based care programs.
  • Has oversight of population health-related policies
  • Maintains an appropriate public relations process that services the best interest of the departments and the community
  • Fosters working relations with the medical profession and other health related facilities/organizations through formal and informal working agreements
  • Reports applicable finding/solutions to Executive Director of Clinics and others as appropriate
  • Engages staff and other stakeholders in continuous improvement of systems and processes

IMPORTANT JOB FUNCTIONS: *

  • Conduct self in accordance with Mahaska Health Partnership’s confidentiality policy and follow all MHP training guidelines in accordance with the Health Insurance Portability and Accountability Act of 1996.
  • Maintain a cooperative working relationship with employees, visitors, medical staff, and patients, keeping in mind the person’s unique needs within their age category (refer to Mahaska Health Partnership’s Age Specific Standards of Care Policy).
  • Attend required in-service programs and staff meetings.
  • All other duties as assigned or requested.

Qualifications: JOB REQUIREMENTS- All Areas:

  • Bachelor’s Degree in Nursing, Public Health or related field required.
  • Master’s Degree preferred.
  • 5 years of experience working in healthcare setting, population health, or related field required.
  • Experience with ACO’s, insurance companies, value-based care, quality programs and initiatives preferred.
  • Experience with quality improvement and project management preferred.
  • Required to show proof of completed Mandatory Reporter Course at time of hire or attend within 6 months of start date.
  • Must adhere to all training requirements as mandated by Federal, State and Professional licensure and regulatory standards.
