JOB SUMMARY: The Director of Care Coordination is responsible for coordinating and supervising the Care Coordination unit of the agency which encompasses CCEVP (Care Coordination for Elderly Virginians Service), options counseling, CRIA (Communication, Referral, Information and Assistance), and Public Information and Education. The Care Coordination Director supervises the agency’s Community Health Worker and is Virginia’s No Wrong Door (aging and disability resource center) liaison, which includes primary responsibility for the internal operation of PeerPlace, Virginia’s No Wrong Door tool.

JOB RESPONSIBILITIES:

  • Supervises and directs Care Coordinators in their provision of CCEVP, options counseling and CRIA.
  • Compiles and reports Public Information and Education activities of the agency.
  • Represents Care Coordination Department with the Virginia Department for Aging and Rehabilitative Services and other social services agencies.
  • Ensures that all programs are in compliance with all applicable rules, regulations and directives of funding sources of the programs.
  • Ensures that the needs of older persons are accurately assessed and that they are properly enrolled in needed services.
  • Designs, arranges, or presents ongoing public benefits and community resources training for the Care Coordination Program or other MEOC staff as appropriate.
  • Keeps Executive Director informed and involved with the progress of Care Coordination departmental activities.
  • Monitors progress on CRIA encounters which require follow-up including options counseling, referrals, etc.
  • Monitors case files.
  • Assists with fundraising.
  • Supervises Care Coordinators in provision of Emergency Services while working closely with the Emergency Services Director.
  • Convenes a No Wrong Door Advisory Council meeting quarterly.
  • Serves as administrator of PeerPlace, Virginia’s No Wrong Door tool, where MEOC client data, referrals, progress notes, etc., are stored.
  • Supervises the agency’s Community Health Worker.
  • Reviews, verifies and approves timesheets, vouchers and claims for reimbursement for staff supervised.
  • Completes an annual evaluation of each department employee.
  • Other duties as assigned by the Executive Director.

QUALIFICATIONS:

  • Bachelor’s Degree, preferably in Human Services related field, or equivalent relevant experience, knowledge and skills.
  • Successful completion of statewide Options Counseling training curriculum and annual updates
  • Successful completion of UAI training
  • Knowledge about issues confronting older adults and individuals with disabilities
  • Understanding of and familiarity with the region
  • Knowledge of community resources, long term supports and funding systems
  • Criminal background checks conducted by the Virginia State Police and Virginia Department of Social Services Child Protective Services Central Registry
