Exodus Recovery, Inc. (Exodus) is looking to hire Crisis Team Members- Discharge Planners for our Access Line Program located in Fresno, CA.

Full time and Per Diem

Position Summary:

This position is responsible for providing care to meet the psychological needs of client through providing indirect services in a manner consistent with Rehabilitation and Recovery Principles. Performs comprehensive screening to clients as well as provident education. Individual will initiate contacts with a variety of community resources; and implement linkages and discharge planning for clients in the Exodus Recovery Urgent Care Center.

MINIMUM QUALIFICATIONS:

• Three years’ clinical practice in psychiatric care with strong emphasis working with the severely mentally ill and dually diagnosed clients.
• Experience with case management with exposure to populations of Chemical dependency and mental health and working within interdisciplinary team and outside community agencies is highly desired.
• Strong knowledge of community resources.
• Strong verbal and written communication skills
• Ability to work independently and shows mature judgment

EDUCATION:

• Master’s Degree required in a mental health related field or nursing (RN license).
• Active registration with the Board of Behavioral Sciences (BBS)

Commensurate with experience.

Job Types: Full-time, Part-time

Pay: $32.00 - $36.00 per hour
