Job description
- Provides individual and group counseling
- Conducts program specific lectures
- Provides case management services on behalf of the client
- Provides discharge/aftercare planning referral services for successful community reintegration
- Collaborates with local/state partners, including volunteers, in order to coordinate effective re-entry services for inmates returning to the community
- Completes clinical assessments to determine course of action and to plan treatment and evaluate the ongoing process of program participants
- Develops and implements pre-release case plans in collaboration with the client, outlining desired discharge outcomes and intervention strategies
- Maintains documentation of all services provided and client contacts made from intake to discharge
- Ensures that client confidentiality and that client rights are maintained
- Participates on a multi-disciplinary treatment team for the purpose of program evaluation, development and case management consultation
- Attends staff meetings and trainings as required

State of South Carolina Minimum Requirements:
A high school diploma and relevant human services or social work experience. A Bachelor's degree may be substituted for required experience. (Or an acceptable equivalency as approved by the Division of State Human Resources).
