Schedule: Flexible
Job Summary: As a Community Living Counselor you will assist individuals with short-term and long-term goal development, maintaining and expanding independence, and community participation.
Responsibilities:

• Empower individuals with personal goal development and skill building
• Support with meal preparation
• Maintain accurate documentation
• Accompany individuals on communityoutings
• Support individuals with home maintenance, social development, financial management, medication management, medical/dental appointments and developing healthy habits
• Other duties as needed

Requirements:

• Ability to support individuals with home maintenance, meal preparation, personal hygiene, and community outings
• A commitment to providing person-centered support
• High school diploma or equivalent
• DHS background clearance
• Valid driver's license, auto insurance, and acceptable driving record
• 18 years of age or older
• Ability to read, write, and communicate in the English Language
• Ability to lift per site requirements
