CHC is seeking a full time Clinical Dietitian to join the team!


The Clinical Dietitian is located at CHC's Riverside practice in Burlington. This role provides nutrition guidance and motivational interviewing, as well as screening, assessments, and ongoing monitoring of patients' nutritional care, documenting processes and outcomes to a diverse patient population. Participates in agency-wide activities pertaining to quality improvement and community education.

Essential Duties

A. Serves as a member of the CHT by delivering patient education and assists clients to set realistic goals for behavior change.

  • Participates in all CHT activities, meetings, trainings.
  • Serves as a critical member of the CHT, working with clients around setting realistic behavior change goals.
  • Coaches clients through behavior change process.
  • Sets up support groups and facilitates client's attendance.
  • Sets up programs specifically around nutrition and physical activity and behavior change.
  • Manages a case load of up to 200 clients.


B. Provides nutrition education to patients and their families.

  • Assesses patient's literacy and cultural needs in selecting education material.
  • Counsels patients and family members using states of change and motivational interviewing techniques regarding nutritional goals and plans to meet those goals.
  • Documents patient and family education in the EHR according to organizational protocols.
  • Maintains inventory of educational materials.


C. Develops and conducts in-services for clinical staff and providers.

  • Updates and develops educational materials to meet patients needs.

Basic Qualifications

  • Bachelor's Degree in Nutrition or Dietetics
  • Dietician License
  • Ability to work with a diverse patient population and work with telephone interpreters
  • Familiarity with electronic medical record
  • Ability to research other cultural customs for appropriate nutritional sensitivity
  • Strong organizational skills and ability to work independently, anticipate needs, creatively problem solve and take initiative to improve patient care at CHC


Compensation commensurate with direct experience within the range of $51,542.40 - $66,060.80 annually.
