Job Profile Summary: Delivers oversight and support for strategic Hierarchical Condition Category (HCC) coding performance and initiatives. Utilizes 3M M*Modal HCC Management tools, identifies, prioritizes, and manages Allina Health Aetna patient HCC diagnosis across the care continuum to support improved documentation quality and accuracy.
Job Description:
Principle Responsibilities
- Prioritizes and Initiates EMR Provider Notifications
- Charts audit visit and diagnosis history of M*Modal prioritized patients to ensure appropriateness.
- Documents opportunities in improving M*Modal identified gaps for inaccurate cases.
- Sends provider notification via EMR pre-visit to initiate accurate HCC capture during visit.
- Uses of M*Modal HCC Collaborate Tool
- Reviews of HCC Collaborate Web-based audit workflow tool to prioritize risk managed patients.
- Supports HCC of risk managed patients.
- Ensures risk capture continuity of risk managed patients.
- Utilizes M*Modal Analytics Tools for Process Outcomes and Performance Communication
- Monitors utilization and compliance through CDS and provider scorecards in HCC Engage and HCC Collaborate.
- Reviews monthly tracking of progress compared to baseline and trends in cohort groups.
- Generates summary level communications for use in meetings, presentations, and newsletters.
- Other duties as assigned.
Job Requirements
- Associate's or Vocational degree with at least 2 years of clinical experience required as a NP, RN, PA or MD or
- Bachelor's degree with at least 2 years of clinical experience as a NP, RN, PA, or MD preferred
- 2 to 5 years clinical experience in hospital/clinic environment required and
- 2 to 5 years experience with Electronic Medical Record, Chart Review and Data Entry required and
- 2 to 5 years experience and high comfort level for working with physicians, nurses, quality leaders and other key stakeholders required and
- NP, RN, PA or MD License with the MN Board required

Additional Job Description:
This position is fully funded until December 31, 2023. There is flexibility in increasing or decreasing FTE to accommodate to the qualified candidate’s current work schedule. If potential candidate prefers a full time schedule, it is with the understanding that the role ends December 31, 2023.
