Exodus Recovery, Inc. is looking to hire a full-time Care Manager for our AB 109 Program. Program offices are located in San Diego (North Park area) and North County (Vista).

Program Overview:

The AB 109 Program serves clients who are under Post Release Community Supervision and Mandatory Supervision, who meet Specialty Mental Health Criteria through San Diego County Behavioral Health Services.

This innovative program provides enhanced strength-based case management services in collaboration with probation and community partners. Individualized treatment plans are based on behavioral health assessment, medication evaluation, and criminogenic needs. Services may include medication management, individual and group therapy, as well as, case management services to provide referrals and linkages to resources in the community. Intensity and duration of supportive services are based on individualized client needs and appropriate levels of care.

Services are provided Monday through Friday 8am – 4:30pm in Central San Diego and/or North County.

Job Summary:

The Care Manager is a clinical member of the multi-disciplinary treatment team and works collaboratively with probation and community partners. The Care Manager is responsible for using a Strengths-Based approach in the delivery of clinically related services from intake to discharge. Assessments include determination of medical necessity with preliminary mental health diagnoses, criminogenic risk/needs factors, and case management needs. Treatment plan development as well as Individual and Group services target mental health, substance abuse, and criminogenic risk factors in a culturally competent, trauma-informed and ethically sound manner. Crisis intervention is provided when appropriate. Treatment and collaboration with the multidisciplinary team and community partners will support clients in their recovery, re-entry into the community, development of healthy and adaptive skills, and reducing the risk of return to jail/prison.

Qualifications/Requirements:

• Education: Master's degree in a mental health related field such as psychology, counseling or social work required.
• License: License eligible with Board registration as ASW, AMFT, APCC (Preferred)
• Valid CA driver's license and good driving record is required.
• Experience: Three (3) years clinical experience in a health sciences related field with a minimum of one-year direct client care experience working with complex clients with SMI who may have multiple conditions including mental health issues, substance use disorders, physical health problems, trauma, and/or homelessness.
• Experience: forensic or criminal justice system (preferred)
• Strong knowledge of behavioral health assessment, therapeutic interventions, community resources and crisis intervention required
• Possess ability to work independently, set priorities and good time management skills
• Strong written and verbal communication skill
• Knowledge of the Risk-Needs-Responsivity model or evidence based treatment for criminal thinking a plus
• Knowledge of County of San Diego or Medi-Cal /CalAIM documentation standards preferred
• Familiarity with CCBH / Anasazi Electronic Medical Record preferred and current access a plus
• Bilingual Spanish preferred

Commensurate with experience.

Covid vaccination preferred.

Job Type: Full-time

Pay: $26.00 - $30.00 per hour
