Job SummaryCoordinates the pastoral/spiritual services for patients and families served by Providence Medical Center and participates in the development of staff regarding the spiritual dimension of health and healing. Serves as a core member of the Hospice Team, the Chaplain will serve as a consultant to the team in area of spiritual needs, acts as a liaison between the team and community clergy/ministers, and provide direct spiritual counseling as directed by the team.
Essential Functions
A. Coordinates pastoral/spiritual services for patients and families including spiritual assessment, necessary documentation, referral processes, facilitating comprehensive response to need and evaluation of efforts. Regular attendance to the weekly patient care conference required.
B. Provides leadership in the development of tools, education of staff and the implementation and evaluation of structures for assessment of pastor/spiritual needs of those served.
C. Assures the response to pastoral/spiritual need is patient centered and comprehensive in addressing issues of health and healing. Identification and development of a response plan for spiritual/pastoral needs, which is thorough, relevant and person-centered.
D. Facilitates the connection of persons to a faith community to provide for ongoing support of health and to assure that patients are connected to faith communities while in our facilities and upon returning home while assuring the confidentiality of patient information. Fosters working relationships with local ministry members.
E. Develops effective working relationships with Providence Medical Center managers to facilitate care in complex cases.
F. Provides leadership in the ministry of prayer and appropriate rituals with patients, families and staff and involves local church partners when possible as a means to raise awareness to the holy and sacred moments in human interaction.
G. Maintains priority in coordinating spiritual/pastoral ministry for those who are dying.
H. Provides in-services and training for all patient care staff in spiritual assessment, loss and grief issues, the meaning of suffering and an appreciation of the spiritual dimension of health and healing.
I. Assists in planning and conducting religious services and programs.
J. Maintains a positive, professional attitude with patients, visitors and other personnel while maintaining patient/family confidentiality.
K. Ability to relate with and work as a team member with other health care personnel, administrative personnel, area clergy and volunteers of varying faith backgrounds. Awareness and acceptance of pastoral expertise; able to consult with others and offer wisdom, insight and knowledge in the professional area.
K. Understand acceptance and application of care coordinator/facilitator principles.
L. Regular attendance at the assigned work location is an essential function of the job.
M. Core Responsibilities of the Chaplain-Certified within the Hospice Department:
1. Attends patient care conference meetings regularly as needed.
2. Develops the initial Spiritual plan of care and revises it as needed, but at least every certification period.
3. Prepares spiritual assessments and clinical progress notes.
4. Provides spiritual counseling to the patient/family as needed.
5. Assists with team members in education to staff.
6. May be called in for spiritual support as available.
7. Contacts patient’s clergy with the family or POA’s permission after initial Hospice spiritual visit to offer support of their services and to alert them to the fact that the patient signed onto Hospice. Encourage the visits from local clergy and other individuals who support the patient’s spiritual needs.
8. Provides documentation of each patient/family or clergy contact.
9. Helps organize Memorial Services and assists with support groups as needed.
10. May assist in providing grief counseling to the patient/family.
11. Works with the social worker and grief counselor on the Bereavement Program.
12. Provides documentation as needed for newsletters.
13. Will not be expected to do anything contrary to personal spiritual beliefs.
14. Maintains patient/family confidentiality.
Education/Experience
1. Bachelor’s degree in theology, behavioral science, ministry or related field preferred.
2. High school graduate or equivalent required.
3. One year experience of pastoral care ministry in an acute care hospital and experience working with terminally ill patients is preferred.
4. CPE (Clinical Pastoral Education) certified by NACC or APC or eligible for certificate is preferred.
5. Experience in community based models of health care, incorporation of integrative therapies for healing and demonstrated success in facilitating comprehensive response plans for pastoral/spiritual care of person preferred.
