onehome is looking for CNAs in the San Antonio, TX area. The Home Health CNA provides assistance with daily living and personal care services in patients homes in accordance with an established care plan and under the supervision of a Registered Nurse or other designated health care professional.

Responsibilities and Duties include, but may not be limited to:

• Traveling to patients' homes to provide assistance with personal care, hygiene, and activities of daily living.
• Encourages client participation in activities to the extent to which the client is able.
• Assists with ambulation as instructed/needed.
• Reports any changes in patient’s condition, living conditions etc. to RN/Supervisor as they occur.
• Complies with all company policies and procedures including, but not limited to HIPPA Privacy rule
• Follows care plan as instructed.
• Accepts only those assignments for which they are qualified.
• Maintain patients' care records and document provided services.

Qualifications:

• Must provide evidence of formal training and/or certification as a CNA as required by state law.
• Must be able to communicate effectively in English, both verbally and in writing. Additional languages preferred but not required.
