Overview:
We are currently seeking a full-time Socialization Coordinator/ Case Manager to join our team at the Golden Gate CONREP program located in San Francisco, California.
Annual Salary: $49,920.00 Responsibilities:
MHM Services, a Centurion company, is proud to be the provider of mental health services to the California Department of State Hospitals Conditional Release Program (CONREP). The CONREP program is a statewide system that provides standardized intensive community outpatient mental health services for judicially committed individuals and mentally disordered offenders.
We are currently seeking a full-time Socialization Coordinator/ Case Manager to join our team at the Golden Gate CONREP program located in San Francisco, California.
Annual Salary: $49,920.00
The
Socialization Coordinator/Case Manager will provide community based services and monitoring to clients that are judicially committed and mentally disordered offenders. The Socialization Coordinator is responsible for providing individualized recreational and socialization support and services, as well as psychoeducation to address coping, life, job, self-care, and social skills.
Qualifications:
- Bachelor’s degree in Social Sciences or Certified Psychiatric Technician preferred but not required.
- Minimum of one year of mental health experience; community-based mental health experience preferred.
- Valid California driver’s license with good driving record.
- Ability to pass drug screen and criminal background check.
Monday through Friday; 8:00am to 4:30pm
