Full-Time Case Manager, Eastern SD (Sioux Falls/Brookings/Madison)
RHD South Dakota Case Management is seeking a full time case manager for Eastern, SD. This position is full time, 40 hours per week. This is a professional level position that will have the ability to work from home; however, travel in coverage area is required. The overall responsibility of this position will be administering the CHOICES waiver services, and coordinating services for adults who have an intellectual or developmental disability. This includes, but is not limited to, coordinating services to meet medical, psychiatric, social, nutritional, educational, vocational, employment, housing, transportation, recreational, legal, and advocacy needs. Case Managers will provide support to individuals to actively engage in and direct the person centered plan development process.
Case Manager will lead person centered planning efforts, focusing on what is important to and important for those on their case load, and designing the supports that will be most effective in achieving a balance between the two. Case Managers will advocate for those on their caseload, assisting with decision making about the supports and services they want and need, and facilitating the provider selection process.
Compensation includes a competitive hourly rate, mileage reimbursement, and initial cost of home office set up.
Sign-On Bonus:
RHD South Dakota Case Management is offering a HIRING BONUS for this position! A $500 hiring bonus to new employees to be distributed in two parts during the first three months of employment. Part one-Following the successful completion of New Hire Orientation hiring bonus will be processed for the employee. The Case Manager will receive $250 gross. Part two-Following the initial 90 days of successful and continuous employment as a Case Manager, a second hiring bonus will be processed for the employee. The Case Manager will receive $250 gross.
Responsibilities/Duties
- Facilitate team process for the initial and annual development of the Individual Service Plan (ISP)
- Ongoing monitoring of ISP and documenting results and intervene as necessary
- Report and review Critical Incident Reports (CIR) per protocol
- Participate and monitor Human Rights Committee (HRC) and Behavioral Intervention Committee (BIC) reviews as appropriate
- Other duties as assigned
- Focus on Personal Outcome measures
- Knowledge and understanding of Charting the Life Course framework and Person Centered Thinking/Planning
- Evaluate the needs of those on caseload in order to remain successful in the community
- Link those on caseload to programs and supports that are needed and requested to meet their needs
- Maintain caseload of individuals with intellectual or developmental disabilities
- Complete required documentation of services, including input of data, reports, and plans into Therap
Required Experience
Qualified candidates will possess a bachelor's degree in human services, social work, sociology, or psychology, and 2 years of experience working in a similar role.Required Education
