Job Summary

As a function of the Waupaca County Health and Human Services Department, the Case Manager works with the Children's Long-Term Support (CLTS) Program to provide family centered assessments, support, and service coordination/case management services to families who have a child/children with developmental disabilities. The focus of this position is to create a safe and stable home and community environment for eligible children and their families.

This position works under the direct supervision of the Family & Community Services Manager


Essential Responsibilities

Conducts family-centered, strength-based assessment focusing on the child and family's strengths and needs within the home.

Manages cases of children with developmental disabilities, physical disabilities, and severe emotional disturbances. Case management includes identifying and connecting families with resources, monitoring overall family well-being, and completing necessary documentation and reports.

Perform data entry into multiple systems.

Adhere to strict timelines in determining functional eligibility for programs and updating individual service plans for children.

Determines financial eligibility for CLTS waivers and/or Children's Community Options Program (CCOP) and all other aspect of Medicaid waiver eligibility.

Advocates for clients and their families to ensure accessibility and service delivery.

Attends staff meetings and trainings as directed.

PERIPHERAL RESPONSIBILITIES:
Performs other duties as assigned by the Family & Community Services Manager.

Qualifications

EDUCATION: Graduation from an accredited college with a degree in social work or human service field.

Preferred: Graduation from an accredited college with a degree in social work or human service field and possess a valid Social Worker Certification.


EXPERIENCE: Preferred experience in working with children with developmental disabilities, physical disabilities, and severe emotional disturbances.

Knowledge, Skills & Abilities

Thorough knowledge and understanding of the impacts of trauma and adversities on brain development. Able to apply trauma informed philosophies into direct practice.

Knowledge and experience working with children with developmental and/or physical disabilities, or children with severe emotional disturbance and their families.

Ability to maintain accurate and detailed records.

Ability to prioritize work with strict timelines.

Ability to follow written and oral instructions.

Ability to communicate effectively orally and in writing.

Ability to establish and maintain effective working relationships.

Ability to maintain agency confidentiality.

Must have available transportation to perform on-site responsibilities.
