What We’re Looking For…
Currently, we are seeking a full-time
Case Manager to join our team! We have a challenging opportunity to provide comprehensive case management for independent living residents within our large continuing care retirement community (CCRC). This position is responsible for helping residents and their families manage difficult life situations by conducting assessments of resident’s needs and level of functioning, collaborating with family and medical providers to develop a plan that addresses needs and identifies strengths and resources as well as coordinating services. This will include assessing a resident’s ability to live in their current setting and if necessary, encourage and facilitate a move to a higher level of care so that dignity, safety and optimal independence can be maintained. The ideal candidate will have a MSW degree and a strong social services background.
