onehome is looking for a Case Manager Assistant.

Job Summary: Assists the Case Managers in ensuring all documentation is completed accurately and in compliance with Plan of Care. Contacts appropriate staff to correct and/or report any deficiencies noted.

What to expect in your role:

• Review assigned medical records to ensure appropriate documentation, quality of care provided, visit utilization, appropriate contacts with physicians, adherence to the doctor’s orders and plan of care.
• Communicate with staff any concerns or deficiencies uncovered.
• Monitor staff with continual deficiencies and report to Administrator/Designee to resolve ongoing problems.
• Analyze data to identify areas for improvement in the quality system
• Identify training needs and organize training interventions to meet quality standards
• Assure ongoing compliance with quality of documentation provided.
• Assure timely submission of all documentation and report any staff continually out of compliance.

What you need to succeed:

• High school degree or equivalent.
• Strong computer skills.
• Good judgment and attention to detail.
• Excellent communication skills.
• Able to communicate effectively in English, both verbally and in writing. Additional languages preferred.

Pay: $15.00 - $18.00 per hour
