Job Description: Continuum Care Manager, RN

On Call Requirement and Response Time: None

Shift/Hours per Week/Schedule:

• Full-Time, 40 hours/week
• 8:30am-5pm with rotation into the weekends

Weekend and holiday requirements:

• One major summer holiday (Memorial Day, 4th of July, Labor Day) and one winter holiday (Thanksgiving, Christmas, New Year's Day) required.
• Every third weekend requirement

Internal Pay Scale: Starts at $71,900

Responsibilities: The Continuum Care Manager is also known as a case manager. They will help with discharge planning by performing a needs assessment and determining what resources the patients will need at discharge. Through the patients' stay at the hospital, they will be responsible for reviews to see if the appropriate level of care is being provided. This will allow a smooth transition between care settings. This is referred to as utilization review. They will also be responsible for identifying and resolving obstacles preventing patients' discharge.

Must Haves:

• License: Current professional Nursing Licensure in the state of IL required
• Education: BSN
• Certification: Basic Life Support for Healthcare providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross).
• Experience: Minimum of 1 year of Care Management experience in an acute setting with Utilization Management Experience & a minimum of 2 years clinical nursing experience in any area.

• Assigned Unit: Continuum Care Manager may work in any unit in the hospital.
• Patient Population and Case Load: Adults. The current case load is 18 patients/day on average. It can range from 16-20.
