Family Home Care is searching for an experienced Care Manager to join our team. The ideal candidate has 2+ years of professional experience in providing hands-on care to elderly and/or disabled adults; has availability and experience for an on-call rotation; is comfortable training & supporting our team of caregivers with hands-on care; is a highly organized professional committed to providing quality care and is passionate about improving the lives of our clientele and employees.

Typical Responsibilities of the Care Manager:

  • Responsible for conducting client intakes / assessments as well as regularly scheduled wellness visits and reassessments after change of condition; of key importance is to enrich lives by finding safe, creative ways to help clients maintain as much independence as possible
  • Responsible for managing schedules for services. Staffing cases with well-matched client/caregiver combinations. Developing and maintaining clear and concise client care plans that include physical, emotional, and social well-being; creating extraordinary, thoughtful client experiences every day. Communicating regularly with clientele and employees regarding service schedules and all schedule changes.
  • Responsible for communicating and collaborating with clients, families, care providers, collaborative partners, and referral sources regularly, ensuring that they are highly satisfied with the care being provided to their loved ones
  • Responsible for field work to include: on-site caregiver training and caregiver support, face-to-face introductions of caregivers to clients and their families, ongoing quality assurance / wellness visits to client's homes, and more.
  • Responsible for supporting day-to-day operations with office team to ensure proper client care and coverage as well as smooth operations and client satisfaction
  • Responsible for providing periodic after-hours support as needed by clients
  • Responsible for ensuring that we continue to be the provider of choice through exceptional care to our clients and by being a true partner to our referral sources; maintaining and enhancing our high standards and best in class reputation

Preferred Qualifications

  • Minimum Experience Requirements: (1) year in care management working with aging population in home health, hospice or home care industry; other professional experience may be acceptable; (2) years of verifiable, professional hands-on care.
  • Ability to interface effectively and professionally with clients & employees, must ability to build and maintain relationships
  • Ability to work in both a collaborative team environment and an independent, self-motivated environment
  • Excellent written and verbal communication skills
  • Valid driver’s license and proof of current auto insurance

Job Type: Full-time

Pay: From $53,560.00 per year
