Elder Life Management currently has an opening for a professional Geriatric Care Manager. The position requires knowledge and ability to assess, plan and arrange care for elder population both at home or in a facility setting. MSW or BSW required, with experience in Geriatrics preferred. Our Care Managers have full use of company cars for all employment-related purposes.

Specific Job duties include:

• Coordinating care based on a comprehensive assessment
• Plan for and attend doctor's appointment as support & advocate
• Problem solving & proactive resolution on an ongoing basis
• Act as a liaison to families at a distance
• Arrange & monitor in-home help or other services as needed
• Provide crisis intervention
• Make recommendations, engage and coordinate community resources
• Enhance quality of life by recommending and arranging meaningful social, spiritual & cultural activities
• Ongoing advocacy
