As a member of our Preventative Health Services team, you'll be responsible for using our Preventative care services software platform to perform eligibility checks, telephonic Health Risk Assessment, and
Annual Wellness visits, enroll Medicare patients and other qualifying patients into Chronic Care management (CCM), while educating the patient on their Advanced Care Plan and potentially offering other services based on the client set up. Each client will have a complete operations book identifying the services they have engaged, the details of their specific workflow and processes (for example where the Care plans are placed into the EMR.) You'll use your health care experience, talents, and problem- solving skills to develop individualized comprehensive care plans using the doctor's notes and data, in office and over the phone care sessions, and work as part of the care team in the practices. Responsibilities Include
- Run eligibility on Medicare patients to see if they are eligible for an Annual Wellness visit
- Complete a Health Risk Assessment over the telephone
- Schedule patients to come in for the Annual Wellness visit
- Complete Annual Wellness visits and Advanced Care Planning documentation.
- Complete individualized patient center care plans and provide chronic care management services using our CCM software.
- Provide feedback to the Clinical Outreach Coordinator, Quality Assurance Nurse, and Clinical
Operations Manager on an ongoing basis as requested/required.
- Telephonic contact with patients to provide care coordination, support, and manage compliance with the Chronic Care Management program.
- Communication to and from Primary Care Clinician or designee in regard to emergent patient needs and or life-threatening episodes.
- Document client communications both verbal and written, in an accurate, efficient, and timely manner.
- Expectation that you will be required to understand the basic functioning of the client’s EHR and
Scheduling systems
- Keep your supervisor informed of all issues pertinent to the care plan process and any known or perceived issues.
- Contribute as a positive member of the Department by supporting all members of the team in a productive, supportive and constructive manner.
- Adherence to documentation protocols and submit daily work logs, escalation of client issues and internal communications.

Skills & Requirements
- Certified Medical Assistant, LVN, RN, MSW license in good standing
- BILINGUAL speaking is a plus
- Must have experience working with Electronic Medical Records (EMR) software
- Demonstrated knowledge of healthcare terms, conditions, roles and basic care principles
- Excellent customer service skills demonstrated by positive feedback from patients and partners.
- Strong organizational and communication skills
- Ability to develop and build trusting relationships with patients and partner practices.
- Work independently on assigned tasks.
- Stay focused on ongoing quality and daily productivity requirements.
- Must be highly motivated, result-oriented with strong skills in communicating, organizing, and time management skills
- Solid problem-solving and consultative skills.
- Excellent computer skills and knowledge.
- Proficient with Outlook, MS Office, phone system.
