About the Job
As a Care Coordinator, you will be appointed as a beneficiary's point of contact for coordinating care and services across the physical, behavioral, dental, developmental, and social service delivery systems, allowing them to receive the correct care at the right time. treatment Coordinators work with patients to ensure that they receive timely and effective treatment from inside clinicians, as well as referrals to community services outside of the clinic. The ultimate objective is to provide well-coordinated treatment and achieve the best clinical results possible. Care Coordinators work with patients individually and as part of a team to achieve these goals, and they are best positioned to educate patients about the health advantages of coordinated care due to their education and expertise.
Duties and Responsibilities:
- Process Care Management and Behavioral Health referrals as directed;
- After obtaining patient consent, complete the appropriate Care Management/Behavioral Health Intake;
- Create/update a Care Plan with patients and/or their supportive friends/family, as well as foster patient participation and follow-up.
- Document all sorts of patient encounters in the medical chart.
- Supports patients' mental/physical/dental health care on a given patient caseload.
- Closely coordinates treatment with the patient's medical, behavioral, dental, and other healthcare professionals as necessary;
- Screening and evaluating people for prevalent mental health and drug use issues as necessary;
- Provides patient education on prevalent mental health and drug abuse illnesses, as well as treatment choices, as advised by a healthcare physician;
- Assists with clinically necessary referrals to evidence-based psychosocial therapies (such as behavioral activation or problem-solving therapy) whether inside the clinic or outside;
- Referrals for clinically appropriate services provided outside of the organization (e.g., social services such as housing help, vocational rehabilitation, mental health specialized care, drug addiction treatment) are facilitated.
- As needed, coordinates with the Community Health Worker (CHW).
- Attends weekly and/or daily Huddles or Case Consultation sessions to identify patient needs and keep the patient's Care Plan and healthcare team up to date;
- Provides relevant community referrals/resources to meet the requirements of the patient;
- Patients are given crisis intervention and referrals to services.
- Collaborates with the Director of Social Services, Clinic Managers, Quality Director, Patient Advocate, and/or HHP Clinical Consultant on the patient grievance process.
- Establishes and maintains collaborative partnerships inside the company and with community partners.
- Attends all required staff meetings, and required department training;
- Follows and supports administrative policies of the agency;
- Participates in continuing education initiatives to maintain continued knowledge and competency.
- Develops links with community service groups;
- Participates in agency performance evaluations on a regular basis while employed.
Qualifications:
- Bachelor’s degree in Social Work, Counseling, Psychology, Nursing, or a related healthcare field (preferred);
- Experience in outreach and inter-agency referral services preferred;
- Previous experience with Epic EMR
- Knowledge of Microsoft Office and Google Suite;
- Knowledge of basic nursing care and procedures, medical terminology, and a strong understanding of HIPAA;
- Current BLS and First Aid certification
- Authorized to work in the US
