NEW PROGRAM - STABILIZATGION AND ASSESSMENT CENTER: Jonathan’s Place is seeking a Behavioral and Aftercare Specialist for the Stabilization and Assessment Center (SAC) Program. This position will work collaboratively with direct care staff in the SAC Program to provide behavior management services. The Behavioral and Aftercare Specialist will provide skills training and support to children/youth with behavioral and emotional issues. The Specialist will be part of a multidisciplinary team providing services and assessments to children/youth to inform future placement decisions. This position will assist in connecting children/youth to ongoing services and provide aftercare linkage and follow-up.
The Behavioral and Aftercare Specialist is a full-time position of 40 hours per week, with evenings and weekends as needed. Jonathan’s Place offers a comprehensive benefits package including health coverage, 401k, and other ancillary benefits.

Responsibilities Include:
- Engages in a trauma-informed programming model to provide residents with physical and emotional safety.
- Notify the supervisor of serious incidents immediately and meet all deadlines outlined in TDFPS Minimum Standards and agency policies and procedures.
- Provides skills training and community-based services for children/and youth with dual-diagnosis of mental health and developmental disabilities.
- Assist children/youth in identifying and articulating his/her requests and needs for services and support.
- Negotiate and facilitate the array of services and supports needed to address the youth’s goals and desired outcomes as identified.
- Provide behavioral intervention services (in the cottage, in-home/community training, and support) to an assigned caseload.
- Responsible for ensuring planned services and supports are implemented and appropriately documented.
- Connect youth with appropriate services and supports.
- Provide TBRI/MAB as a de-escalation technique.
- Provide feedback to admin and direct care staff regarding the resident.
- Offer support to direct care staff and program staff for crisis management, including de-escalation, behavior management training, behavior intervention, modeling appropriate crisis strategies, and conducting resident observations.
- Collaborate with the treatment team to create and address the individualized needs of youth in the program.
- Meet designated benchmarks and or/ quality measures.
- Able to work weekend shifts (as needed)
- Provide aftercare planning and support for children and youth in the program.
- Demonstrate a high level of integrity and confidentiality.
- Attend all job-related training.
Required Education and Experience:
- Bachelor’s Degree or higher in Social Work, Psychology, or Human Services.
- QMHP experience and QMHP Certification a plus
- One to three years of experience in Crisis Management or Behavior Intervention
- Two to three years of experience working with children/youth with behavioral and emotional issues.
- Knowledge of child growth and development, developmental disabilities, and autism
